Development of health science popularization works for the prevention of deep vein thrombosis in patients after joint replacement

Objective To develop evidence-based health science popularization works for the prevention of deep vein thrombosis(DVT) in patients after joint replacement. Methods In this study,guided by the Guideline International Network patient guideline formulation manual and Patient Education Materials Assessment Tool,we conducted semi-structured interviews to clarify the health science themes. These themes were matched with relevant evidence,and the relevant evidence recommendation items for each theme were refined. It is translated into easily understandable language and other forms. 12 representatives(doctor,nurse,patient,science popularization expert and evidence-based methodology expert) conducted an external review of the quality of these science popularization works. Results Prevention of post-operative DVT,patients’ main concerns and the most urgent need for action were extracted from a total of 6 scientific topics. After the matching with 22 pieces of evidence,a health education content system for the prevention of DVT after total joint arthroplasty was created. According to the system framework and evidence,as well as the preferences and needs of patients,a popular science manual and popular science video were created,with the final external review rate of 94%~100% and the coefficient of variation of 0~0.101. The revised health science works for the prevention of DVT after joint replacement included popular science manuals-trifolds and popular science videos. The copyright of both popular science manuals(copyright number:Guozuo Dengzi-2022-L-10159159) and popular science videos(copyright number:Guozuo Dengzi-2022-I-10162339) have been registered. Conclusion The method of developing science popularization works on the prevention of DVT in patients after joint replacement is scientific. It can provide a methodological reference for the development of other science popularization works.

Construction and evaluation of a risk prediction model of postoperative delirium in patients with Stanford type A aortic dissection

Objective To construct a risk prediction model of the postoperative delirium for patients with acute type A aortic dissection(ATAAD),and to verify the prediction effect of the model. Methods A retrospective study was conducted among 245 adult patients undergoing aortic dissection surgery in the cardiac surgery department of a tertiary A hospital in Wenzhou from October 2020 to April 2021. According to the occurrence of postoperative delirium,the patients were divided into a delirium group (n=51) and a non-delirium group (n=194),and the data of the 2 groups were compared. The risk prediction scoring model was constructed based on the multivariate logistic regression analysis of ATAAD postoperative delirium risk factors. The Hosmer-Lemeshow test was used to determine the model’s goodness of fit of the model,and the receiver operator characteristic curve (ROC) was used to test the model differentiation. The risk prediction model was validated by the prospective study with inclusion of 51 ATAAD patients admitted to the cardiac surgery department from July,2020 to June,2021. Results There were 6 factors included in the risk prediction model of patients with postoperative delirium,namely age over than 55.50 years(1 point),male(1 point),preoperative blood lactic acid value more than 1.85 mmol/L(1 point),hypothermia cycle arrest time more than 36.50 minutes(1 point),length of stay in CCU(cardiovascular intensive care unit)more than 8.50 days(2 points),and other postoperative complications(1 point). The Hosmer-Lemeshow test of the prediction scoring model showed that when P=0.393,the area under the ROC was 0.926. When the patient’s score was ≤4,the incidence of postoperative delirium in patients with ATAAD was≤15.63%;while it was≥5,the incidence was ≥50.00%. The sensitivity,specificity and accuracy of the risk stratification model were 76.47%,93.30% and 89.80%,respectively. The result of model verification shows that when the area under the ROC was 0.896,the Hosmer-Lemeshow test P=0.359. The sensitivity,specificity and accuracy of the risk stratification model were 75.00%,92.31% and 88.24%,respectively. Conclusion This risk prediction model provides an effective prediction effect,and it is simple and convenient to use in clinical application,which could be a reference for medical staff to take preventive treatment and nursing care.

Expert consensus on risk assessment and prevention of venous thromboembolism in general surgical patients

Objective To explore the expert consensus on risk assessment and preventive nursing of venous thromboembolism(VTE) in general surgical patients. Methods This consensus was drafted through a systemic literature review on VTE prevention and the interviews of nursing experts. The draft was thereafter reviewed by a total of experts in related fields,and it was finalized after 2 rounds of expert consultation. Results 4 domains with 26 components were established through a modified Delphi method. The effective recovery of the ques-tionnaire was 100%;the expert judgment coefficient was 0.960;the familiarity was 0.912;the authority coefficient was 0.936. The Kendall coefficients of the first and second level topics in the 2 rounds were 0.149 and 0.261,and 0.122 and 0.203,respectively,with statistically significant differences(P<0.05). The consensus includes risk factors for VTE in general surgical patients,risk assessment,risk response,education and quality control,and it is applicable to perioperative patients of general surgery. Conclusion The consensus was based on the recommendations of 25 clinical nursing experts. The results are scientific,rigorous and authoritative,and can provide guidance for clinical nursing staff in the prevention and control of VTE in general surgical patients.

Expert consensus on safety management of transfusion connection devices for vascular access

Objective The aim is to establish an expert consensus(hereinafter referred to as "Consensus")on safety management of transfusion connection and port protection devices for vascular access,to prevent catheter related bloodstream infection and provide patients with safe,effective and standardized intravenous therapy. Methods Using evidence-based methods,according to the level of evidence,the evidence in this field was searched,evaluated and summarized,and relevant recommendations and research conclusions were extracted,and then the first draft of consensus was formed. Through a round of Delphi expert consultation and 2 rounds of expert meetings,opinions from domestic experts in many fields and disciplines were sought and integrated;the content was adjusted and the consensus was reached. Results The positive coefficient is 100%;the authoritative coefficient is 0.92;the average value of each index is more than 4;the coefficient of variation is less than 0.22. The Kendall’s harmony coefficient of the expert’s opinion is 0.35( χ2=144.44,P<0.001). A consensus recommendation on the classification and maintenance of infusion connection devices was reached. Conclusion The Consensus is scientific,which can provide a reference for the safety management of transfusion connection and port protection devices in China,and provide a basis for clinical decision-making to prevent catheter related bloodstream infection.

Research on influencing factors of usage intention toward internet diagnosis and treatment services among enterostomy patients

Objective To investigate the influencing factors of enterostomy patients’ usage intention of Internet diagnosis and treatment services within the theoretical framework of Unified Theory of Acceptance and Use of Technology and Dual-Factor Model. Methods Using a convenience sampling method,310 enterostomy patients were recruited from 3 tertiary hospitals in Hefei city from March to September 2021. The general data questionnaire,m-eHEALS Scale and the questionnaire on the usage intention of the Internet diagnosis and treatment services of enterostomy patients were used for investigation. Structural equation model was constructed to analyze the influencing factors. Results The score for usage intention was(3.37±1.31),with the performance expectation of (4.08±0.76),effort expectation of (3.11±1.33),social influence of (3.66±1.02),facilitating conditions of (3.80±1.06),trust of(4.06±0.58),resistance to change of(3.07±1.29),and ehealth literacy of(2.53±1.07). The results of the structural equation model show that social impact,effort expectation,and ehealth literacy have positive effects on the usage intention,with effect values of 0.251,0.230 and 0.082,respectively. Resistance to changes negatively affects the usage intention,and the effect value was -0.506(P<0.05). Conclusion The level of usage intention of Internet diagnosis and treatment service of enterostomy patients is in the middle of the upper. Resistance to changes,social influences,effort expectations,and ehealth literacy are the key factors affecting usage intention of enterostomy patients. To enhance patients’ usage intention of Internet diagnosis and treatment.

Objective To analyze the situation and hot trend of venous thromboembolism(VTE) prevention abroad,in order to provide references for clinical guidelines and scientific research for nursing staff in China. Methods Core journal articles related to venous thromboembolism and prevention from Web of Science(WOS) from 2005 to 2020 were retrieved. CiteSpace software was used for the bibliometric and cluster analysis. Results The volume of papers issued in the research of VTE prevention is on the whole increasing trend. The research mainly focuses on drug prevention,physical prevention,complications observation,risk identification and VTE prevention in patients with different diseases. Among them,“precautionary duration” “intermittent pneumatic compression devices” “stroke”“anticoagulation” “clinical practice guidelines” and “oral anticoagulants” are the hot spots in the current research frontier. Conclusion The prevention of VTE is attracting the attention of scholars. It is recommended to promote the development of VTE prevention research in China and continuously improve the quality of clinical care by specifying the focus of VTE prevention research,broadening the range of VTE prevention populations,and formulating VTE preventive care guidelines.

Objective To explore the nursing human resources management strategies of the medical aid team to Hubei during the coronavirus disease 2019(COVID-19) epidemic and to improve the efficiency of nursing management. Methods Drawing on the command and decision-making system of health emergencies,and taking "emergency support-basis-action" as the conceptual model of nursing manpower management,we constructed a scientific,effective and practical ICU nursing manpower management model. Results There were 135 members in nursing aid team with 7 nursing groups being established to implement flexible management. Progressive trainings were carried out and clinical teaching was conducted for 30 times. A total of 109 critically ill patients with COVID-19 were admitted,including 75 patients with mechanical ventilation. Conclusion In response to the outbreak of COVID-19,the national medical aid team quickly established ICU nursing team,and formed a management model of nursing manpower based on "emergency support-basis-action" model to protect the critically ill patients with COVID-19 and to ensure the patients treatment and care being carried out smoothly.

Objective To describe the development and evolution of venous thromboembolism(VTE) nursing research in China,and to reveal research hotspots and frontiers. Methods Journal articles from CNKI and WanFang Database from the inception to 2019 were searched and retrieved with keywords of venous thromboembolism and nursing. CiteSpace software was used for the bibliometric and cluster analysis. Results The volume of papers issued in the nursing research of VTE is on the whole increasing trend. The direction of VTE nursing research involves the identification of high-risk groups and the development of nursing preventive measures. Among them,the elderly,the patients after joint replacement,the patients with malignant tumor and the patients after PICC catheterization have always been the research focus of the prevention of VTE. Conclusion As one of the specialized nursing direction,the nursing care of patients with VTE has been given more attention. Future research can explore effective methods in order to improve nurses’ knowledge for prevention of VTE,and develop intervention programs to promote the “flawless” VTE prevention

Objective To strengthen the standardized training of nurses and management of key links to ensure the multiple needs of epidemic rescue,training and personnel safety in the context of the outbreak of coronavirus disease 2019 (COVID-19) epidemic. Methods Based on hospital information platform,the nursing service department has strengthened management system,training system,prevention and control system. We have carried out reasonable and deep division of labor,coordinated grid management;scientific assessment of epidemic situation,dynamic human adjustment;special online training,to ensure the coverage of all staff;timely evaluation and assessment,focusing on practical exercises;real-time trend management,effective isolation management;full attention to humanity,and good psychological adjustment. Results 46 nurses were transferred in hospital wards to help decrease the infection risk during the epidemic. 4 special online trainings were completed,and the average training participation rate was more than 90%;the satisfaction rate of trainings was more than 95%. Due to daily on-line report mechanism,36 nurses reported their contact history with confirmed cases and then were isolated at home,and now all of them have been released from the quarantine. Conclusion The targeted management strategies during the epidemic can effectively guarantee the work safety of the training nurses,improve the cognition and ability,and ensure the standardized training in an orderly,efficient and safe manner.

Objective To establish an expert consensus on nursing prevention and control of venous thromboembolism(VTE) in adult patients with coronavirus disease 2019(COVID-19). Methods This consensus was drafted through a systemic literature review on COVID-19 diagnosis and management plan,VTE prevention and control,and interviewing nursing experts from front-line epidemic control. The draft was thereafter reviewed by a total of 18 experts in related fields. This consensus was finalized after 2 rounds of discussions and modifications. Results The expert consensus covers the following 5 topics,namely VTE risk factors,VTE risk assessment,VTE nursing prevention and control,VTE prewarning and monitoring,and health care education of VTE prevention and control after discharge. Conclusion The consensus was drafted and finalized on a scientific and practical basis,which can provide guidance for clinical nurses in prevention and control of VTE in adult patients with COVID-19.

A multicenter randomized controlled study on removal indication of peripheral venous indwelling needle

Objective To investigate the clinical safety of pulling peripheral venous indwelling needle according to clinical indications,and compare its effect with conventional indwelling needle extraction. Methods A multicenter randomized controlled trial was conducted with a total of 3,669 patients in 12 tertiary general hospitals from March 2019 to July 2019. The patients were randomly allocated into the observation group(group A) and the control group (group B) by the random number table. The puncture methods were consistent between the two groups. Researchers compared the indwelling time,the incidence of catheter-related complications within 96 hours,and the risk ratio of catheter-related complications during the entire indwelling period(phlebitis,occlusion,and infiltration,suspected infection) between two groups,among which phlebitis was the primary outcome indicator and other complications as secondary outcome measures. Results A total of 3,642 patients were enrolled in the study with 1,803 cases in group A and 1,839 cases in group B. The indwelling time of group A was(85±52)h and it was longer than that of (71±30) h in group B with a statistical difference(P<0.001). Within 96 h of indwelling time,there was no statistical differences in the risk of phlebitis,infiltration and suspected infection between two groups(P>0.05). During the entire indwelling period,the risk of occlusion in group A was higher than that in group B with a statistical difference(P<0.05). The other indicators showed no statistical difference. Conclusion The placement of indwelling needle based on clinical indications can prolong the indwelling time without increasing the risk of phlebitis,infiltration and suspected infection,but the risk of occlusion will increase. If the catheter is removed according to clinical indications,monitoring and evaluation of occlusion should be strengthened.

Investigation on occupational exposure and protective behaviors of nurses at emergency departments in tertiary hospitals in Beijing

Objective To understand the occurrence and protection status of three occupational exposures:needle-stick injury by nurses,blood exposure and workplace violence in the emergency department in tertiary hospitals in Beijing. Methods According to the schedule of the emergency department of each hospital,one group of nurses were selected from each hospital in 21 tertiary hospitals in Beijing to participate in the online survey. Self-designed online questionnaires were distributed and completed anonymously,including general information,the occurrence status and protective behaviors of needle-stick injury by nurses,blood exposure and workplace violence. Results A total of 1,150 questionnaires were collected,of which 1,102 were valid,with an effective rate of 95.83%. In the past year,the incidence rates of at least one needle-stick injury by emergency department nurses,blood exposure during blood collection,verbal violence,physical violence and sexual harassment were 37.8%,36.4%,68%,12%,and 5.3%,respectively. In different working areas at emergency department,the occurrence of needle-stick injury(χ 2=16.805,P=0.01),verbal violence(χ 2=98.509,P<0.001) and physical violence(χ 2=79.916,P<0.001) were different. Among nurses with occupational exposure,92.8% were able to squeeze blood after needle-stick injury,flush with flowing water and disinfect;69.8% were able to flush and disinfect exposed parts after blood exposure in the process of blood drawing. Among nurses encountering workplace violence,41.3% explained patiently,and 30.0% adopted tolerance and avoidance. In addition,the reporting rate of all three types of occupational exposure was low(less than 30%). Conclusion The incidence of occupational exposure in emergency department is relatively high. Nursing managers should attach importance to the construction of the work environment in emergency department,strengthen the training of pre-exposure protection in blood collection process and workplace violence,and strengthen the reporting and management of the three types of occupational exposure.

Objective To standardize the maintenance of clinical venous catheter,so as to reduce the catheter related complications,prolong the service life of catheter and reduce medical cost.Methods The consensus was formed through literature searching,interview with key stakeholders,and repeated expert discussion meetings.Results The consensus summarized the latest evidence in six chapters covering all aspects of catheter maintenance,such as flushing and locking tube,dressing replacement and catheter maintenance,infusion joint,catheter removal,education and training,and infection control.Conclusion Items in this consensus are concise and easy to implement,and the new content can supplement and perfect the existing norms and standards to a certain extent,which is helpful to solve clinical problems and guide clinical practice.
